We are required to have parents or guardians complete a Withdrawal Form if they are transferring to a private school or moving out of District. Please complete each section with the current grade your child is in on the withdrawal form below and send it to Victoria Friedman (Email Victoria Friedman). Please complete the Release of Records with your student's new school that they will be attending. When you email it, please send it as a PDF format . If you require further assistance with the withdrawal form, please contact Victoria Friedman.
